
Living with diabetes can mean seeing different health professionals and keeping track of appointments, tests and ongoing care. The Multidisciplinary Team (MDT) Program is a free service designed to make this easier by bringing your healthcare providers together to support you and your health.
The program works alongside your existing healthcare providers - it does not replace them.
When you join the MDT Program, you’ll have a dedicated Allied Health Clinical Lead who will help coordinate your care and be your main point of contact.

You can be referred by an RFDS doctor, primary health clinic nursing staff, an NWRH allied health clinician, Lives Lived Well, a Diabetes Educator or a Healthy Outback Communities (HOC) Community Connector. You can also refer yourself.
